Algal Oil vs. Fish Oil: Is the Vegan Source Actually Equivalent?

Arterburn 2008 ran algal-oil DHA capsules head-to-head against cooked salmon and found them bioequivalent: DHA rose the same amount in plasma phospholipids (~80%) and erythrocytes (~25%) in both groups. Maki 2014 matched fish oil's triglyceride-lowering effect almost exactly at high doses (18.9% algal vs 22.9% fish oil, not significantly different). But no algal-oil product has run a hard cardiovascular-outcome trial — VITAL, ASCEND, REDUCE-IT, and GISSI-Prevenzione all dosed fish-derived EPA/DHA. That's the honest ceiling on "equivalent," not a reason to distrust the biomarker equivalence that IS well established.
The bioequivalence evidence: as close to a direct test as exists
The strongest single piece of evidence in this comparison is Arterburn 2008 (n=32, randomized, open-label, parallel-group, 2 weeks): healthy adults took either algal-oil DHA capsules or assayed portions of cooked salmon, both delivering 600mg DHA/day. DHA increased ~80% in plasma phospholipids and ~25% in erythrocytes in both groups, and the difference between groups was statistically equivalent by a post-hoc bioequivalence analysis — the authors' own conclusion: algal-oil DHA capsules and cooked salmon "appear to be bioequivalent." A companion trial, Arterburn 2007 (n=12/group, randomized, placebo-controlled, 28 days), dose-ranged two different algal DHA oil strains (200/600/1,000mg DHA/day) plus a DHA-fortified food against plasma and erythrocyte DHA response: all three delivery forms produced statistically equivalent, linear, dose-dependent uptake, with excellent tolerability and no serious adverse events. Matched triglyceride-lowering at high doses — despite very different ratios
Maki 2014 is the cleanest head-to-head algal-vs-fish-oil trial in this evidence base, and the ratio contrast makes the equivalence finding more interesting, not less. The algal arm used a DHA-heavy 2.7:1 DHA:EPA blend at 2400mg/day; the fish-oil comparator used a much more EPA-heavy 0.7:1 DHA:EPA (EPA-heavy) blend at 2000mg/day. Despite the different compositions, both arms produced statistically significant, statistically indistinguishable triglyceride reductions (18.9% vs 22.9%, both p<0.001 vs. control). That's a genuine equivalence finding at matched high doses — not proof that ratio never matters (see the dosage guide for when it does), but proof that a DHA-heavy algal blend and an EPA-heavy fish oil blend can produce the same triglyceride outcome at a high enough dose. LDL-C rose in both active arms (+4.6% algal, +6.8% fish oil, both p<0.05 vs control) — the next honest thread.
The LDL-rise pattern: shared, not algal-specific
It would be easy to read the LDL-C increase in algal-DHA trials as a mark against the vegan source specifically. The evidence doesn't support that framing. Geppert 2006 (114 vegetarians, EPA-free algal DHA) found total and LDL cholesterol both rose significantly alongside a real triglyceride benefit. Sanders 2006 (n=79, algal DHA+DPA) found total, LDL-, and HDL-cholesterol all rose significantly at a high dose. And the largest, least industry-connected source in this evidence base — Bernstein 2012, a Harvard-affiliated meta-analysis pooling 11 RCTs (n=485) — confirmed the pattern is consistent across the algal-DHA literature, not a one-off finding: pooled LDL-C rose +0.23 mmol/L (roughly +9mg/dL) alongside triglyceride and HDL benefits. Crucially, Maki 2014's fish-oil arm raised LDL-C too (+6.8%, actually numerically larger than the algal arm's +4.6%). High-dose DHA raising LDL-C is a pattern that shows up with marine-derived DHA generally, from either source — a real trade-off worth disclosing every time the triglyceride benefit is mentioned, and not a reason to single out algal oil as the worse option.
The one-line takeaway On absorption and lipid biomarkers, algal oil and fish oil behave the same way — bioequivalent DHA uptake, matched triglyceride-lowering at high doses, and the same LDL-rise trade-off. The honest ceiling: no algal-oil hard-outcome trial exists
Every cardiovascular-outcome trial that news coverage typically cites for omega-3s — VITAL, ASCEND, REDUCE-IT, and GISSI-Prevenzione — dosed fish-derived EPA/DHA, not algal oil. Algal oil's evidence case rests on two pillars that are both genuinely solid: bioequivalence (it raises blood DHA the same way fish-derived DHA does, per Arterburn) and biomarker/lipid movement (it moves triglycerides, LDL, HDL, and the omega-3 index comparably to fish oil, per Maki, Geppert, Sanders, and Bernstein). The extrapolation from those two pillars to "algal oil prevents cardiovascular events the same way fish oil does in some trials" is biologically reasonable — the body doesn't know or care which organism made the DHA it absorbs — but it has never been directly tested. State the claim at the level the evidence supports: algal oil raises blood levels the same way fish oil does. Don't extend it to a hard clinical outcome no algal-oil trial has measured.
