Iron is the one supplement you should test for, not guess at. Most people get enough from food, and taking iron you don't need is genuinely harmful — your body has no way to dump the excess. If a blood test shows you're low, two things matter more than the brand: the dosing schedule (taking it every other day often absorbs more than daily) and finding the cause. For the form, gentle bisglycinate is easiest to tolerate over the months it takes to refill your stores.

The counterintuitive part: taking iron every day can absorb less

Here's the finding that overturned decades of "take it three times a day" advice. When you swallow a dose of iron, your body responds by raising a hormone called hepcidin, which sharply reduces iron absorption for the next day — fractional absorption of a second dose fell by roughly 35–45% (Moretti 2015, PMID 26289639). So if you take iron again the next morning — or twice the same day — that second dose lands while the door is still partly closed, and more of it passes through unabsorbed, irritating your gut on the way.

The fix is counterintuitive: take iron once every other day. In iron-depleted women, alternate-day single doses absorbed a higher fraction of iron per milligram than consecutive-day or twice-daily dosing (Stoffel 2017, PMID 29032957; Stoffel 2020, PMID 31413088). The honest nuance: total repletion ends up similar either way — a 2024 systematic review found equivalent hemoglobin recovery on daily versus alternate-day dosing, with fewer side effects on alternate days (Kamath 2024, PMID 37979057), and the 2024 AGA practice update frames every-other-day dosing as mainly a tolerability win with similar overall absorption (DeLoughery 2024, PMID 38864796). So the practical wins are a gentler schedule and not wasting iron — not necessarily faster correction.

The practical protocol. Which form: ferrous sulfate vs bisglycinate (and the elemental-iron trap)

First, the number that actually matters: elemental iron, not tablet weight. Iron salts are only partly iron — a 325 mg ferrous sulfate tablet is only ~65 mg elemental iron (about 20%). Always read the elemental figure on the Supplement Facts panel.

On the form itself, the honest verdict is narrower than the marketing. Ferrous sulfate is the cheap, effective, most-studied standard — but it significantly increases GI side effects (constipation, nausea, dark stools) versus placebo (Tolkien 2015, PMID 25700159). Iron bisglycinate ("gentle iron") is a chelated form that resists food and mineral inhibitors and tends to leave less irritating free iron in the gut. It matched ferrous sulfate at a lower elemental dose in a pregnancy trial (Milman 2014, PMID 24152889), and a 2023 meta-analysis found fewer GI effects and higher hemoglobin in pregnant women — but no advantage in children (Fischer 2023, PMID 36728680) — and, as with many supplement trials, several bisglycinate studies are industry-funded. So the "2–3× better absorbed" claim overstates it: bisglycinate is a genuinely gentler option, not a magic upgrade.

Side effects — and how to reduce them

The common complaints are constipation, nausea, stomach cramps, and dark stools (the dark color is harmless and expected). They come mostly from unabsorbed iron irritating the gut, which is why the fixes all point the same way: take iron every other day rather than daily (fewer doses, less irritation), use the lowest effective elemental dose, and switch to bisglycinate if a standard salt makes you miserable. Taking it with a little food eases nausea at a small absorption cost. If you get black and tarry or bloody stools, that's not the supplement — see a doctor.

Signs your iron might be low

Iron deficiency often builds quietly. Common signs include unusual fatigue, pale skin, shortness of breath on exertion, cold hands and feet, brittle nails, more hair shedding than usual, headaches, and restless legs or a craving to chew ice (pica). These overlap with many other conditions — including B12 deficiency — so treat them as a reason to get a ferritin test, not to self-diagnose and start iron. A blood test is the only way to know.

Who actually needs iron

Iron requirements vary more by sex and life stage than almost any nutrient. The RDA is 8 mg/day for men and post-menopausal women, 18 mg/day for menstruating women (19–50), and 27 mg/day in pregnancy — all as total intake from food and supplements combined. Most people meet this through diet. The groups who actually run low, and may need a supplement after testing, are:

  • Menstruating women — monthly blood loss makes this the most common deficiency group.
  • Pregnancy — demand rises sharply; supplementation is often clinician-directed.
  • Endurance athletes, frequent blood donors, and people with GI conditions — higher losses or lower absorption.
  • Vegetarians and vegans — plant (nonheme) iron is less absorbable, though a well-planned diet can still meet needs.

Treating a real deficiency (and finding its cause)

If a test confirms you're low, treatment is a months-long project, not a casual habit. Two things trip people up. First, there are two finish lines: hemoglobin (the anemia) usually rebounds within weeks — you feel better — but ferritin, the storage tank that prevents relapse, lags far behind and needs 3–6 months of consistent iron to refill. Keep going past feeling better and recheck ferritin, not just hemoglobin, before stopping.

Second, and more important: iron deficiency is a symptom with a cause. In menstruating women and pregnancy the reason is usually obvious. But low iron in a man or a post-menopausal woman is a genuine red flag — it commonly points to gastrointestinal bleeding, from a benign ulcer to, occasionally, colon cancer. Supplementing without finding the source can mask a problem that needs treatment. If your deficiency has no obvious explanation, getting it investigated matters more than the supplement.

Safety: why iron is different from every other supplement

There is no exit valve for iron. Your body can absorb iron but has no active way to excrete a surplus, so chronic over-supplementation accumulates as iron overload, which damages the liver, heart, and pancreas over time (people with hereditary hemochromatosis reach this faster). Acutely, iron is a leading cause of poisoning fatalities in young children — a handful of adult tablets can be lethal, so keep iron locked away. Iron also binds several drugs (levothyroxine, some antibiotics), so separate them by a few hours. None of this applies to iron you actually need — it's the argument for testing first, not for fearing a diagnosed course.
